Sohema Functional Overview

 

Sohema is purpose-built for health and human services case management. It collects information involved at each stage of a case in the workflow and activities of your organization. Using Sohema, caseworkers, clinicians, and other authorized users conduct online assessments to evaluate client needs. A variety of available assessment tools are available with the application, and assessment templates are also included to develop additional assessments unique to your organization's needs.


Case Planning
Using both assessment results and data captured at intake, Sohema can help in generating case plan recommendations and determine eligibility for specific programs and services. 

 

Case Monitoring and Outcomes Measurement
Once eligibility has been determined, Sohema allows you to capture all services provided to each client with associated case worker, open/close dates, outcomes, service status, types of service, associated histories, and wait-listing. Sohema not only maintains a history of case managers, services, statuses and effective dates, but also links services directly to provider information. Sohema stores all service utilization data and tracks all program outcomes.
